Request for the information

According to the Law of Ukraine “On the Access to Public Information”, public information is the one that is displayed and documented by any means and on any carrier, which was received or created by the state bodies while they are performing their functions, prescribed by the legislation, or, which is in the possession of the governing bodies. 

State Enterprise “Medical Procurement of Ukraine” provides information as a response to the request for such information. 

Request for the information should contain: 

  1. Name of the person that applies, post address or e-mail, phone number; 
  2. General description of the information needed or the type, name, details or content of the requested document, if this information is available for the requester; 
  3. Signature and date, if the request is sent in writing.

Citizens' Appeals 

According to the Law of Ukraine "On Citizens' Appeals", citizens of Ukraine have the right to apply to public authorities, local governments, associations of citizens, enterprises, institutions, organizations, regardless their ownership, media, officials, in accordance with their functional responsibilities, with remarks, complaints or suggestions regarding their statutory activities. This might as well include requests for the realization of their socio-economic, political or personal rights and legitimate interests, or a complain on their violation.

Citizens' appeals should be understood as written or oral proposals (remarks), statements (petitions) or complaints.

The appeal can be submitted by an individual (individual appeal) or by a group of persons (collective appeal).

The appeal should include: 

  1. Name, surname of the person that applies, postal address or e-mail, phone number; 
  2. Description of the proposal, statement or complaint; 
  3. Signature and date, if the request is sent in writing. 

An appeal submitted in writing, without indicating the postal address and not signed by the author/authors, as well as the one where the author cannot be identified, is to be considered anonymous and will not be reviewed.

Notes: 

  1. By submitting a request, the requester automatically agrees to the processing and use of his/her personal data in accordance with the Laws of Ukraine. 
  2. Request forms can be received at the State Enterprise "Medical Procurement of Ukraine", 22 Khreshchatyk Str., 2nd floor, Kyiv, 01001.
  3. The request should contain information on where and in what form the answer should be given. 
  4. Answer on the request for information is given in the way and form chosen by the requester, within five working days from the date of receipt of the request.
  5. In cases when request for information concerns information that is used for protection of person’s life or freedoms, state of the environment, quality of the food or household items, accidents, catastrophes, dangerous natural phenomena or other emergencies that have occurred or may occur and threaten safety of citizens, the answer should be given within 48 hours from the date of receipt of the request.
  6. In case when request concerns provision of the large volumes of information or requires search of information, the period of consideration of the request may be extended to 20 working days with justification for such extension. The requester shall be notified of the extension in writing no later than within five working days from the date of receipt of the request. 
  7. Information is provided free of charge. 
  8. If the requested information contains documents longer than 10 pages, the requester shall be notified on this within five working days from the date of receipt of the request, indicating the amount of actual costs associated with copying or printing documents, and details and procedure for reimbursement such costs. The information should be provided within three working days after receiving confirmation of payment of the actual costs.
  9. The request may be denied in the following cases:

9.1. State Enterprise "Medical Procurement of Ukraine" does not own and is not obliged, in accordance with its competence provided by the Statute, to be the holder of the information on which the respect was made; 

9.2. The requested information belongs to the category of information with limited access or is confidential; 

9.3. The applicant has not paid the costs associated with copying or printing documents 

9.4. The requirements for submitting a request for information as mentioned in p 5., Article 19 of the Law of Ukraine “On Access to Public Information” have not been complied, namely: 

  • Name of the person that applies, postal address or e-mail, phone number; 
  • General description of the information needed or the type, name, details or content of the requested document, if this information is available for the requester; 
  • Signature and date, if the request is sent in writing.
